Job Summary
The Medical Assistant focuses on patient outreach and care coordination, handling both the front-of-house (administrative) and back-of-house (clinical) operations.
Pay: $19-22/hr

Core Responsibilities
Conduct initial outreach to recently discharged members.
Take and record vital signs, medical histories, and patient information.
Assist in billing and coding.
Order and keep inventory of medical supplies.
Enter and maintain accurate patient records in electronic medical systems.
Educate patients on preventive care, medication usage, and post-treatment instructions.
Schedule appointments and manage patient flow.
Prioritize outreach based on clinical needs and post-acute risk factors.
Determine the appropriate intervention pathway
Confirm discharge details, evaluate immediate needs, and assess potential barriers to care.
Coordinate with the Care Coordination team to ensure visits are scheduled within required timeframes (24–72 hours).
Support members with education, planning, and navigation during early post-discharge periods.
Ensure accurate documentation in TCC’s care platform and maintain program compliance.
Assist in building and improving TOC workflows, SOPs, and process standards.
Collaborate across internal teams to improve operational effectiveness.
Qualifications
Associates degree or Medical Assistant certification
1-3 years of experience in medical assistant or adjacent role.
Strong organizational, multitasking, and problem-solving skills.
Excellent verbal and written communication skills, with empathy for patients.
Comfort using scheduling platforms, Microsoft Teams, email, and digital communication tools.
Ability to coordinate transportation logistics and provide patient support.
Self-motivated, detail-oriented, and able to work independently in a remote environment.
No specific licenses required, though prior healthcare experience is highly preferred.

Ability to communicate clearly via phone, email, or video platforms.
