Compensation & Benefits
Hourly pay: $28 - 30/hr based on experience
Benefits package for full-time employees: health, dental, vision insurance; paid time off; 401(k)/retirement; disability.
Mileage stipend, company-supplied devices / technology, onboarding / community paramedic academy training.
Position Summary
The Community Paramedic delivers high-quality in-home clinical care as part of Total Care Connect’s Mobile Integrated Healthcare programs. This role focuses on proactive patient care for high-risk populations, supporting hospital readmission reduction, emergency department avoidance, and improved chronic disease management. Community Paramedics conduct scheduled patient visits, perform clinical assessments, deliver care within established protocols, and collaborate with virtual providers and interdisciplinary care teams to provide coordinated, patient-centered care.
Community-Based Clinical Care
Conduct scheduled patient visits within community-based care programs.
Perform comprehensive patient assessments including vital signs, symptom evaluation, medication reconciliation, and environmental safety review.
Deliver care within established clinical protocols and standing orders.
Provide treatments such as wound care, medication administration, and point-of-care testing when applicable.
Identify early signs of clinical deterioration and escalate concerns through clinical or operational pathways.
Care Coordination & Patient Support
Support continuity of care following hospital discharge or acute medical events.
Assist patients in understanding care plans and follow-up appointments.
Identify barriers to recovery including environmental or social determinants of health.
Collaborate with care coordination teams to support patient stabilization and long-term health management.
Documentation & Communication
Complete accurate and timely documentation within the electronic patient care record system.
Ensure documentation supports clinical accuracy, medical necessity, and payer requirements.
Maintain communication with operational teams regarding visit updates and patient needs.
Participate in documentation reviews and quality improvement initiatives.
Required Qualifications
Current Ohio Paramedic certification in good standing
Minimum 2–3 years of paramedic field experience
Valid driver’s license with acceptable driving record
Strong clinical assessment and patient education skills
Experience using electronic documentation systems
Work Environment
Field-based role providing care in patient homes, partner offices, or community clinical settings.
Travel between patient homes, partner offices, and operational locations within the assigned region
Exposure to varied home environments including pets, limited lighting, clutter, stairs, and environmental hazards.
